Exam Form

This form is for students enrolled in the exam track or competition training program for the purpose of collecting complete information for those preparing the second practical exam. Fill out all questions completely. An accompanying fee is required for those wishing to participate in the Honors Recital (see below). Those taking part only in the exam and not in the Honors Recital do not need to pay this fee.

To participate in the Honors Recital, note the requirements as published in the 2023-24 Agreement, namely:

• Participation for this event is by audition only, and restricted to students currently enrolled in the Exam Track or Competition Preparation program.
• Students must demonstrate a successful history of performance during the 2023-24 academic year prior to this event, which may include recitals, festivals, or other activities deemed appropriate by the student’s primary instructor (Note that participation in performance classes only, or non-CAM organized events unrelated to the area of performance, such as choir, pop music event, etc, do not qualify as acceptable activities.).
• The student must be recommended to perform at this function by his or her primary instructor.
• Eligible students must participate in both exam dates, the second of which serves as the audition.
• Participants must be accepted via live audition only (No video or audio submissions are acceptable for students intending to participate.).
• Results of the audition to perform in the Honors Recital are final and may not be appealed.
• All students must also be prepared to perform their musical selection by memory.
• An accompanying $75.00 participation fee accompanies this application for those wishing to perform in the Honors Recital (see below). Those participating only the exam need not pay this fee.
• The event is free of admission to the listenership and open to the public.
• Participants and their families must stay for the duration of the event (one’s own recital session; roughly 90 min).

For those participants who wish to receive a 2023-24 Diploma certifying successful completion of all year-end requirements, this is distributed at the end of the Honors Recital. Further criteria to be bestowed a diploma include the following additional criteria:

• complete all requirements set forth by the student’s primary instructor (set by the end of September, 2023);
• participate in both exams in January and June;
• successfully perform in two or more recital events (not including the Honors Recital);
• successfully participate in one or more outside festival or competitive event (those who have not yet participated in one but still plan to apply for the Virtuoso Artists Festival on June 17 may include this future festival on the list below);
• attend and observe two or more live professional classical concerts during the academic year (note the free guest artist recital co-sponsored by the CAM featuring Robert Brooks Carlson on June 2 at 2 pm at Forest Ridge School of the Sacred Heart);
• be accepted AND participate in the Honors Recital.

Graduating seniors who have received three consecutive diplomas leading up to graduation receive a Chopin Academy of Music Performance Diploma.

If you are planning to attend the June 9 Honors Recital at Benaroya Hall, payment of the $75 participation fee must be made through the following link. Please include the name of the performer (same as the first field of this form). A separate fee must accompany each performer of the recital.

List up to two pieces you wish to perform for the exam. The first required row must contain the piece you wish to play in the Benaroya Hall Honors Concert (if you plan to participate). Provide movement inscriptions (Allegro, Agitato, etc). List composer name in column 2, and precise duration in minutes and seconds in the third column.
